If your child will need to take any medication during the upcoming school year, please be aware a "Medication Authorization" form must be filled out and signed by a doctor. This is for All medications), Prescriptions and /or Over the Counter). If your child has any medical problems including: Diabetes, Asthma, Seizures, Allergies requiring an Epi Pen, etc, please contact Nurse Dee to discuss an emergency plan of action for your child.
